Submit this form to the help desk when an agency needs access to a child already in COPA

OVER INCOME REQUEST FORM
Submit this form in a request to the help desk to enroll an over income child for HS/EHS/EHS-CCP.


ATTENDANCE ANALYSIS FORM
Complete this form and submit to your monitoring team supervisor when a program’s daily average attendance falls below 85% within any quarter.


HEAD START ELIGIBILITY (HSEV) FORM
After enrolling a child in COPA, agency staff should print this COPA generated form. It must be signed and kept in child’s file along with eligibility supporting documentation.


ATTENDANCE ADJUSTMENT REQUEST FORM
Attendance should be completed DAILY. The window to edit attendance for each month closes on the 6th business day of the following month. If an edit or correction is needed after the window has closed, an agency may submit a request using this form. It should be noted that late requests for edits are reviewed and approved on an individual basis.


ENROLLMENT & ELIGIBILITY FILE REVIEW
This form is used by the DFSS monitoring team to review delegate agency enrollment and eligibility documents for accuracy and compliance. Agencies may use it for self-monitoring.
